- Naya Mati — GP consultation with Dr Shiv Chande — forty-five-minute clinical intake on day one plus a mid-week review to fold in the Nidana bloodwork results as they come back. Medical history, medications, cardiovascular and metabolic risk, and the clinical framing that shapes the two Focuses the guest chooses at intake.
- Prakriti Mati — Ayurvedic consultation with Dr Prasanna Kerur — full-hour arrival consultation covering dosha typing, pulse diagnosis, current imbalances, and the Ayurvedic reading of the bloodwork; mid-stay treatment adjustment session; and a departure consultation to hand over the take-home Ayurvedic protocol.
- Pathya Mati — Nutrition consultation with the dietitian — forty-five-minute intake with the HCPC-registered dietitian covering eating pattern, food-and-mood history, and the two-week take-home dietary protocol aligned with the guest's dosha typing. Second thirty-minute session on the last day to review what's shifted across the week and finalise the take-home plan.
- Manas Mati — Psychology consultation with the Manas clinician — hour-long session with the HCPC-registered clinical psychologist covering nervous-system pattern, stress load, sleep-and-mood interaction, and a written frame that guides which of the Focuses (typically Klesha, Manas, or Nidra) would land best if selected.
- Nidana Tier 2 — Medilab Longevity Baseline panel — meaningfully deeper than the Weekend's Tier 1. Advanced lipids (ApoA, ApoB, Lp(a)), fasting insulin with HOMA-IR, extended thyroid, full nutrient work-up, sex-specific hormone panels — markers an informed guest expects to see and an NHS GP typically does not run. Bundled with a thirty-minute Medilab clinician consultation covering results and written protocol.
- Deha — DEXA scan — gold-standard body composition and bone density scan in the dedicated Deha room, run by an IR(ME)R-certified DEXA operator (per IR(ME)R 2018 the operator does not need to be an HCPC radiographer for this scanner class). The baseline every future return visit measures against.
- Kaya — Movement quality baseline with single-plate + Theia3D motion capture — Functional Movement Screen, grip strength on a clinical dynamometer, single-plate power read on the Vald ForceDecks Mini, Theia3D 3D markerless motion-capture of gait and squat with kinematic L/R asymmetry, and two 60-minute Bala training sessions across the stay. The kinesiology scientist reads all findings together and writes the take-home movement prescription.
- Nidra — Orion sleep tracking + Sleep Disruption Test + Terra sleep report — dual-zone temperature-controlled bed with contact-free nightly tracking of HRV, sleep stages, breathing rate and snoring across all five nights; Orion Sleep Disruption Test delivered on the first night to establish the personal sleep-fragility profile; take-home personal sleep report at departure with a two-week home protocol.
- Hima — Two to three cryotherapy sessions across the stay — whole-body LN2 chamber at −140°C, two-to-three-minute sessions delivered by a trained operator. Positioned after Bala training or before the evening wind-down; deeper cadence than Weekend to let recovery-and-sleep effects compound across the week.
- Prana — Two to three guided contemplative sessions — 45-minute guided pranayama and mindfulness sessions with the yoga team across the week, drawn from the Ayurvedic tradition. Enough cadence to open the practice into something the guest can take home and develop rather than a single introduction.
- Written Ayurvedic bodywork prescription — Dr Prasanna Kerur's suggested treatment sequence (abhyanga, shirodhara, udvartana, dosha-specific bodywork) delivered in writing at the end of the arrival Prakriti consultation and updated at the mid-stay follow-up. Treatments themselves are not included in the £1,790 fee: guests book them separately via the Ayush Spa at reservation or on arrival. Specific recommendations are surfaced only on request.
- Up to two Focuses — the guest picks up to two from the eight available (Kanti, Nidra, Klesha, Manas, Kaya, Bala, Agni, Upavasa), booked at intake so the schedule accommodates them properly. Each Focus adds a targeted deep-dive layered on the spine above; shared elements between Focuses are deduplicated at booking. See the Focus architecture section below.
- Two health coaching sessions — one mid-stay working session with the Health Coach to check in on how the week is landing and adjust; one departure session to translate the week's findings into a written four-week take-home protocol. Plus two scheduled post-stay video check-ins across the following month.
- Sauna, steam and hydrotherapy pool — part of the evening wind-down ritual in the Ayush Spa wing.
- Two Long Walks on the Jersey coast with a member of the team — guided, usually late afternoon, at different points across the week.
- Full board at Ahara — breakfast bundled in the room rate; lunch and dinner from the longevity menu at £90 per person per night.
Where each part of the £1,790 actually goes.
The Long Pause is sold as a single bundled fee — £1,790, all-in. £310 is true third-party pass-through — Medilab's Tier 2 Longevity Baseline panel (now including the six standard-inclusion markers folded in on the Nidana review — homocysteine, omega-3 index, free T3+rT3, SHBG) plus the Orion Sleep Disruption Test negotiated per-guest rate. The remaining £1,480 stays with the hotel and funds two blocks of hotel-delivered work: the Sama Clinic clinical spine (£575 — all four Mati consultations, DEXA, Kaya battery with single-plate + Theia3D + 2 Bala sessions, Nidra tracking + Sleep Disruption Test interpretation, Hima cryotherapy across the stay, Prana practice arc) and the coaching / protocol / follow-up wrap (£150). Total hotel delivery cost: £725. Net contribution to new investment: £755 per booking — a 51% margin on the net-to-hotel residual. Ayush spa treatments sit outside the bundled fee — guests receive a written prescription from Dr Prasanna Kerur and book any treatments they want directly through the spa at retail brochure rates. The £110 sticker lift from the earlier £1,680 is held (rather than reversed) — the recovered spa pass-through moves Pause from a genuinely thin 31% to a healthier 51% margin without changing the guest headline.
Medilab and Orion are the true third-party pass-throughs. Sama Clinic clinicians and the Wellness & Movement team are hotel-employed (see the Sama Clinic workforce table and the Staff workforce page); their time is costed at approximate fully-loaded hourly rate from the workforce salary bands. Equipment amortisation for each new item (DEXA scanner, Orion, force plates, cryotherapy chamber) sits on the Equipment page. Focus fees for the guest's chosen two Focuses are billed separately at the guest's chosen selection and are not in this base cost card; each Focus has its own economics — see the individual Focus pages linked from the Focus architecture section below. Ayush spa treatments sit outside the bundled fee — the Prakriti prescription is included; the treatments themselves are booked à la carte through the Ayush spa at retail rates. The 51% margin at the £1,790 fee is the honest Pause with the full four-Mati Sama Clinic spine plus the six new Nidana standard markers, once the spa pass-through recovers as clinical margin.
What price does each margin imply?
Costs above are fixed: £310 of pass-through to Medilab and Orion, plus £725 of hotel delivery (full Sama Clinic spine + coaching / protocol / follow-up, at Programmes-page baseline clinician rates). Ayush spa treatments are outside the bundled fee and therefore outside this calculation. The only thing that changes with a different target margin is the price the guest pays. Drag the slider.

At 5 nights, the medical-clinic set only offers pro-rated packages — most start at 7-night minimum. Long Pause still leads on total clinical density (8.5 filled cells) but the lead narrows once the categories we deliberately don't offer are included in the honest comparison — Palazzo Fiuggi reaches 7 on the back of Fiuggi's thermal-water heritage plus IV and osteopathy, SHA reaches 6 on strong IV and manual-therapy programmes, Lanserhof 5.5. What our proposition does not include at 5 nights: colon hydrotherapy (sits outside the evidence base we're prepared to stand behind), natural thermal / mineral waters (geography — we don't have Fiuggi's aquifer or Park Igls's Tyrolean springs), IV nutrient drips (aesthetic-medicine adjacent rather than longevity-medicine per Dr Shiv Chande's clinical view), and dedicated osteopathy (a workforce hire not yet made — earmarked for Phase 03). Where the argument still holds: we're the only 5-night option with a full 4-Mati spine and Focus architecture, and we sit level with SHA on price while comfortably below Lanserhof, Palazzo Fiuggi, and Six Senses.

Every Long Pause includes the same core clinical spine. On top of that, a Pause guest may add up to two Focuses — Kanti (skin), Nidra (sleep), Klesha (stress and burnout), Manas (depth), Kaya (movement), Bala (performance), Agni (gut), or Upavasa (fasting) — to specialise the week in a specific area. How the guest picks
Focuses are chosen at booking, not at arrival — Medilab needs eight to ten working days to prepare the added panels, and Dr Alexa Kerr's aesthetic diary or Dr Marie-Christine Dix's stress-and-burnout list may need calendar space. There are no defaults; reception offers the eight Focuses on the booking call and confirms the guest's active choice on the pre-arrival questionnaire. Common Pause pairings are Klesha + Nidra (the stress-and-sleep couple), Agni + Klesha (gut-brain), or Kanti on its own for a guest arriving specifically for skin work. A guest may also elect zero Focuses and stay spine-only.
The deduplication rule
Some Focuses share elements. Klesha and Nidra both include a CBT-I course with the Manas psychologist. Klesha and Kanti both draw on the DUTCH hormone panel. When two chosen Focuses overlap, the shared element is delivered once and the second Focus is discounted by that element's cost — the guest is never billed twice for the same test, session, or protocol. Multi-Focus pairings therefore come in below the sum of their sticker prices; reception surfaces the actual bundle price at booking.
How a Focus reshapes the day-by-day
The five-night itinerary below is the spine — every Pause guest goes through it. A chosen Focus adds specific slots on top rather than replacing anything. A Klesha guest picks up a DUTCH cortisol panel at the arrival draw, sees the Manas psychologist for four to six one-to-one sessions across the week (typically two on days two and three, two mid-week, one wrap on day five), and runs the CBT-I sequence in the evening slots. A Kanti guest adds a Dr Alexa Kerr skin consultation on day two, two Aruna red-light sessions in the afternoon rotation, and a hydrafacial or medical peel late in the stay. An Agni guest replaces the standard longevity dinner on two evenings with the kitchari monodiet and takes home the 5R supplementation programme. Dr Prasanna Kerur shapes the written Ayurvedic treatment prescription to accommodate the Focus — sequencing abhyanga earlier in the week if Kanti calls for skin-recovery time, recommending more restorative treatments alongside a Klesha guest, and more stimulating ones alongside a Bala or Kaya guest — for the guest to book against at the Ayush spa if they choose.
Two structured departures from the standard shape — a guest who has recently completed a Reset or View can book one or two Focuses without the Pause spine, and any guest can bring recent external diagnostic results (within twelve months for movement-sensitive markers, eighteen for DEXA) to receive a recent-test credit against the programme fee for the skipped tests. How a Long Pause would run.
Five nights, arriving on a Sunday evening and leaving on a Friday afternoon. Designed so that a guest can take one clean week away from work without it disrupting the rest of life. The shape settles into a rhythm by the second day: an Ayush spa slot or clinical touchpoint in the morning, coaching or clinical work before lunch, movement and rest in the afternoon, early supper, full sleep protocol overnight. Ayush spa treatments are not included in the programme fee — guests can book any treatment at any point during the stay (or pre-book at reservation) via the Ayush Spa, against the written prescription Dr Prasanna Kerur hands over at the arrival Prakriti consultation.
